YOU ARE HEREBY NOTIFIED that the State of Oklahoma has filed Petition for Termination of Parental Rights requesting the court terminate your parental rights to the above named child(ren). A copy of the Petition is attached to this notice.
FURTHER, you must appear before the District Court in and for Stephens County, Oklahoma, Room #309, Stephens County Courthouse, Duncan, Oklahoma, on the 23 day of September, 2026, at 9:00 o'clock a.m. when the Court will hear the merits of the Petition for Termination of Parental Rights and grant such relief or make sure orders as the Court deems necessary and proper in this case.
FURTHER, that in the event that you fail to appear as summoned, the Court will receive testimony and evidence produced by the State at the time of the hearing and will proceed to grant such relief and make such orders as the Court deems necessary and proper, as supported by the evidence and merits of the cause. FAILURE TO PERSONALLY APPEAR AT THIS HEARING CONSTITUTES CONSENT TO THE TERMINATION OF YOUR PARENTAL RIGHTS TO THIS CHILD(REN). IF YOU FAIL TO APPEAR ON THE DATE AND TIME SPECIFIED, YOU SHALL LOSE ALL LEGAL RIGHTS AS A PARENT TO THE CHILD(REN) NAMED IN THE PETITION OR MOTION ATTACHED TO THIS NOTICE.
Upon proper evidence presented and after a finding of fact that it be in the best interest of the child(ren), the court may terminate for all time any and all parental rights you may have to the child(ren); except that termination of parental rights shall not in any way terminate the duty to support the child(ren) until the court receives notice from the Department of Human Services that a final decree of adoption has been entered and the instant case is dismissed.
YOU ARE FURTHER ADVISED that you are entitled to be represented by an attorney at each and every stage of these proceedings. If a state of indigence exists, you are entitled to an appointed attorney at public expense. ALLEGED DEPRIVED CHILD AS DEFINED BY THE LAWS OF THE STATE OF OKLAHOMA
PETITION ADJUDICATE AND TERMINATE ON GREGORY WILSON
COMES NOW, Jason M. Hicks, District Attorney, as Petitioner and for cause of action alleges and states that D.W. is a deprived child under the age of 18 years. Further, the State moves to terminate the parental rights of Gregory Wilson of D.W. pursuant to 10A O.S. § 1-4-904. In support of this Petition, the State of Oklahoma states as follows:
D.W., 4 years old, having been born on the 29th day of August, 2021
The respondent Juvenile(s) are not receiving proper parental care and guardianship and the said Juvenile(s) appear to be DEPRIVED in this, to-wit:
A finding that a parent who is entitled to custody of the child has abandoned the child. Title 10A O.S 1-4-904(2). The father abandon the child and the Department of Human Services has been able to locate Gregory Wilson.
Further, that neither the Indian Child Welfare Act, Title 25 U.S.C. § 1901, et seq., nor the Oklahoma Indian Child Welfare Act, Title 10 O.S. § 40, et seq., is relevant in this matter because the respondent juvenile is not eligible for enrollment with an Indian tribe.
Further, this Court has jurisdiction over this matter pursuant to the Uniform Child Custody Jurisdiction Enforcement Act (U.C.C.J.E.A.) Title 43 O.S. § 551-101 et. seq., and Title 10A § 2-2-102.
That the said Juvenile is in temporary custody and under the control of the DEPARTMENT OF HUMAN SERVICES, Duncan, Oklahoma.
That the names and addresses of parents, legal guardians, or near relatives in the absence of parents or legal guardians, are as follows:
FATHER TO O.W.: GREGORY WILSON
WHEREFORE, premises considered, your Petitioner respectfully prays: that this matter be set for trial to adjudicate D.W. as a deprived child and terminate the parental rights of Gregory Wilson, and such summons and notice be given as required by law.

That upon such trial, said juvenile be adjudged DEPRIVED and be dealt with according to law; that proper intermediate and temporary relief be granted; that each parent/legal guardian be required and ordered to pay child support for said minor while the said juvenile is not in their custody.
That termination of parental rights of the Gregory Wilson to D.W., and placement of the child outside of the home is in the best interest of the child, and that reasonable efforts have been made to prevent the need to remove the child from the home.
That the Parent's failure to appear at the hearing shall constitute a denial of interest in the child, which denial may result, without further notice of this proceeding or any subsequent proceeding, in the granting of the application for the termination of your parental rights and in the child's adoption.
WHEREFORE, the State of Oklahoma prays that this Court finds D.W. to be adjudicated as a deprived child as to the Gregory Wilson and that pursuant to Title 10A O.S. § 1-4-904 the Court find that the parental rights of the Gregory Wilson should be terminated, and that said child be placed with a suitable person or agency for the purpose of finding a suitable home for adoption and for the further purpose of consenting to the adoption of said child. And that all other proper relief be granted.
